What is Gout?

Gout is the most common form of inflammatory arthritis in men over the age of 40, and it affects approximately 3 times as many men as it does women.  There has been an increase over the last 40 years of newly diagnosed cases.

Gout is a type of arthritis which has been in existence since ancient times.  Hippocrates called it "the disease of the kings" due to the fact that it was generally associated with a rich diet.  However, in reality, there are many different factors that can lead to gout, and diet is part of this larger picture.

The pain that is experienced with gout is caused by inflammation when needle-like crystals are deposited in connective tissue or in the fluid that cushions a joint.  These crystals are made up of uric acid, which is a substance that is produced when the body breaks down purines that are found in the human tissue and many of the foods that we consume.  A majority of uric acid is carried through the bloodstream to the kidneys.  It is here whereby the kidneys eliminate the uric acid from the body in the urine.  However, if the body produces too much uric acid, or if the kidneys do not eliminate a sufficient amount of it, it can build up in the blood.  This condition is referred to as hyperuricemia which can, in some cases, lead to attacks of gout.

There are 4 types or stages of this disease:

    * Interval
    * Chronic Tophaceous

All of the above can be dangerous if left untreated, and they vary in levels of pain from discomfort to extreme pain and complete body immobility.

An attack of gout will generally begin suddenly, and bring symptoms of severe pain, tenderness, redness, warmth, and swelling in the large joint of the big toe.  The other joints that can be affected are the instep, ankles, heels, knees, wrists, fingers, and elbows.  On rare occasions, the shoulders, hips, or spine may be affected.  The attack with generally subside after 3 to 10 days, and the next attack may not occur for months or even years.  However, gout attacks do become more severe over time, as well as last longer and affect more than one joint.  The possibilities are also that the attacks will happen more frequently.

The symptoms of gout vary, as they can occur after an illness or surgery.  Gout can also appear first as nodules on the hands or ears.

Gout can become so severe that it eventually causes damage to the joints, which can potentially lead to disability.  However, when the correct treatment is undertaken, most people with gout are able to control the symptoms.  Different medications are available to treat gout which will ease the painful attacks and prevent future attacks.  Medication will also assist in the prevention of long-term damage to the joints and related disability.

Over two thousand years ago Hippocrates, widely considered by western civilization to be the father of modern medicine, referred to Gout as "the disease of kings." This is because of the correlation between gout, a rich diet, an excessive consumption of alcohol and a sedentary lifestyle.


Traditional Gout Treatments

Gout generally attacks a single joint powerfully and suddenly, most commonly the big toe.  Joints that are affected by gout show the most visible red signs of inflammation of any of the 100 types of arthritis.

Gout patients are generally 95% men, and 5% women.  An initial attack may last for several days and disappear, even when untreated.  Following attacks may not occur for weeks, months, years, or even not at all.  Repeated attacks may eventually cause damage to the joints and loss of mobility.

Gout is often related to an inherited abnormality in the body to process uric acid.  Levels of uric acid can increase when you eat foods that contain purine.

Treatment goals include terminating acute gout attacks, safe relief of pain and inflammation, preventing future attacks, and avoiding complications in the future.

Though gout is generally treated successfully, it can become more of a challenge if other conditions exist along with gout, or if the patient does not comply with the recommended lifestyle changes, or the medication regimen.

It is highly recommended to alter your dietary habits, such as avoiding foods that are rich in purine.  Other preventative measures include an increase in your water intake, weight loss, a reduction in your consumption of alcohol, and using the correct medication.

    * Non-steroidal anti-inflammatory drugs (NSAIDS)
    * Colchicines
    * Corticosteroids
    * Adrenocortocotropic hormone (ACTH)
    * Allopurinol

1. NSAIDS are commonly the first medication to be prescribed, especially indomethacin.  Other NSAIDS may be equally effective.  This type of medication is prescribed at maximum dosage and reduced as the symptoms of gout decrease.  The medication should be taken until the pain and inflammation are non-existent for at least 48 hours.

2. Colchicine is used for treating acute flares of gout, and to prevent recurrent attacks.  This medication does not cure gout; rather it prevents or relieves gout attacks by reducing inflammation.  This medication may be used in two ways:  it can be taken in small quantities for months or years, or taken in large quantities for a short period of time.

3. Corticosteroids or adrenocorticotropic hormone (ACTH) may be taken by patients who cannot use NSAIDS or colchicines.  ACTH is given in the form of an intramuscular injection over several days.

4. Allopurinal is prescribed for those patients with chronic gout.  It works by affecting the system that manufactures uric acid in the body.  This medication is used to prevent attacks of gout, rather than treat them once they occur.

5. Probenecid is prescribed for chronic gout.  It is used to prevent attacks, rather than treat them once they occur.

6. ColBenemid is a medication that contains Probenecid, which is a uricosuric agent, and Colchicine, which has anti-gout properties.

7. Sulfinpyrazone is also known as a uricosuric agent and it is used to treat gouty arthritis.  This medication lowers the quantity of uric acid in the bloodstream, which prevents gout attacks.

Other important factors to take into account for preventing gout are:

* Drink plenty of water.
* Maintain a healthy weight.
* Make alterations to your diet, such as avoiding          purine-rich foods and reducing your alcohol consumption.
* Be compliant with the treatment that is recommended by your doctor.
